Terms of Use
These terms cover your use of FormOasis (formoasis.com), run by WP Mantis LLC (“we”, “us”). Using the site or buying a template means you accept them. The short version: the calculators are free and give estimates, not advice; templates are licensed to you for your own sites and your clients’ sites; and if a template doesn’t work as described, we make it right.
The calculators
The calculators are free to use and need no account. Each result is an estimate based on the numbers you enter and the formula shown under “How it’s calculated” on that tool. Results are for information only. They are not financial, tax, legal, medical or other professional advice.
Before you make a decision that matters, such as taking out a loan, changing your diet or planning a pregnancy, check the numbers with a qualified professional who knows your situation. We work to keep every formula correct, but we don’t promise that any result is accurate, complete or right for your circumstances, and rates, tax rules and guidelines change. If you spot an error, tell us through the support page and we will fix it.
Buying a template
Each template page lists what you get, the price and the plugins it needs, before you pay. Payment is handled by Stripe, and prices are in US dollars. Your download link appears right after payment and is emailed to the address you give at checkout. Keep your own copy of the files.
Plugins you need
Templates are files for Gravity Forms. They do not include Gravity Forms or any add-on, such as GP Advanced Calculations. You need your own licenses for those, bought from their makers and used under their terms.
WordPress, Gravity Forms and add-ons change over time, so a template that works when you buy it may need adjusting after a future update to one of them. We help where we reasonably can, but we don’t control those products.
Your license
When you buy a template you get a non-exclusive, non-transferable license to:
- use it on any number of sites you own or build for clients;
- change it however you like.
You may not resell, share or give away the template files or the PDF guides, whether on their own or inside another product such as a template library, theme or plugin. A client site that uses the finished form is fine; handing the files to someone else to reuse is not. We keep ownership of the files; what you buy is the license to use them.
Refunds
Digital files can’t be returned. If a template doesn’t import, or doesn’t work as described on its page, tell us within 14 days of purchase and we will fix it or refund you in full.
The free guides and site content
The build guides, tool pages, formulas, text and design of this site belong to us. You are welcome to follow the free guides to build forms for yourself or your clients. Please don’t copy or republish the guides, tool pages or calculators elsewhere, scrape the site, or frame or embed our pages in another site without asking first. A short quote with a link back is fine.
Ads and affiliate links
Ads and affiliate links keep the tools free. If you buy through an affiliate link we may earn a commission, at no extra cost to you. We only link to plugins we use ourselves, but your purchase is with that seller and under its terms. Ad and affiliate placements are labeled.
Using the site fairly
Don’t try to break, overload or probe the site; get around payment for downloads; scrape it with automated tools; use its forms to send spam; or use it for anything unlawful.
Ideas you send us
If you suggest a tool or an improvement, we may use the idea freely, without owing you anything for it.
Other sites
Links to other websites are there for convenience. We don’t control those sites and aren’t responsible for their content, products or practices.
Trademarks
FormOasis is an independent resource. Gravity Forms, Gravity Perks, WordPress and other product names belong to their owners, and mentioning them doesn’t mean they endorse this site.
No warranty
The site, the calculators, the guides and the templates are provided “as is” and “as available”. To the extent the law allows, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose and non-infringement. We don’t promise the site will always be available or free of errors.
Limits on our liability
To the extent the law allows, we are not liable for any indirect, incidental, special or consequential loss, or for lost profits, data or business, arising from your use of the site, a calculator result or a template. Our total liability for any claim is limited to the amount you paid us in the 12 months before the claim, or $50 if you haven’t bought anything. Some places don’t allow these limits, so they may not all apply to you.
Your responsibility
If your use of the site or a template breaks the law or these terms, and someone brings a claim against us because of it, you agree to cover our reasonable costs of dealing with that claim.
Governing law
These terms are governed by the laws of the State of Arizona, and any dispute will be handled in the courts located in Arizona. Before going to court, please contact us so we can try to sort it out directly.
Changes
We may update these terms as the site changes. If a change matters, the date below moves and the change is described here. Purchases are covered by the terms in place when you bought. Last updated October 23, 2026.
